Jason A. Brooks, M.D. received his Indiana State Medical License in April 2008. Dr. Brooks graduated with his Bachelor’s Degree from Purdue University in West Lafayette, Indiana in 1999 and obtained his Medical Degree from the Indiana University School of Medicine in Indianapolis, Indiana in May of 2004.
Dr. Brooks completed his Post Graduate Training with a Transitional Internship at the Ball Memorial Hospital in June of 2005 and an Ophthalmology Residency at the Indiana University School of Medicine in Indianapolis, Indiana in June of 2008.
Since 2008 he has proudly been a Physician at Munster Eye Care Associates, P.C.
Dr. Brooks holds a Professional Membership with:

- American Academy of Ophthalmology (Member)
- American Society of Cataract and Refractive Surgery (Member)
- Indiana State Medical Association (Member)
Dr. Brooks holds hospital affiliations with Community Hospital and St. Catherine Hospital; he also holds active status at Community Surgery Center and Munster Same Day Surgery.
Dr. Brooks resides in Munster Indiana and speaks English. He has been a member of Northwest Indiana for over 30 years.
